Life & Disability Insurance
LIFE INSURANCE
Life Insurance provides additional financial protection for you and your family in the event of your death or serious accident. Sight & Sound Ministries, Inc. provides all employees 150% of your annual pay to a maximum of $150,000 in Life and AD&D insurance. Benefits are reduced by a certain percentage as an employee ages: 35% at age 65 and 55% at age 70.
SHORT-TERM DISABILITY
Short-Term Disability through Guardian provides you some financial relief during a time of temporary or permanent disability. Your benefit amount is 60% of your basic weekly earnings to a maximum of $1,500 a week. The maximum benefit duration is 11 weeks of payment (4 weeks for a routine pregnancy/6 weeks for a cesarean section). The elimination period (amount of time you must be out of work prior to being eligible for an STD claim) is 14 consecutive days. Regular full-time and contracted cast employees are eligible for this coverage.

As an employee who will be receiving Short-Term Disability benefits, you have the option to use PTO hours in coordination with your disability benefit to bring your weekly income to 100%. Learn more.

VOLUNTARY LONG-TERM DISABILITY
Long-Term Disability through Guardian provides financial relief after Short-Term Disability ends, on day 90. This voluntary benefit is 60% of your monthly earnings to a maximum of $7,000 a month.
VOLUNTARY LIFE INSURANCE & AD&D
Offered by Guardian, this coverage enhances the limit provided by Sight & Sound.
Employee Coverage

Choose from $20,000 to $500,000, in $10,000 increments.

Guaranteed Issue: Up to $200,000 (under age 65) the first time you are eligible for coverage (no health questions required).

Benefits reduce by 35% at age 70 and 55% at age 75.

Amounts above the Guaranteed Issue + Additional Amount require an Evidence of Insurability (EOI) form.
Spouse Coverage

Choose from $10,000 to $250,000, in $5,000 increments.

Cannot exceed 50% of your elected employee coverage.

Guaranteed Issue: Up to $50,000 (under age 65).

Spouse coverage terminates at age 70.
Child Coverage

Choose from $2,000 to $10,000, in $1,000 increments.

Cannot exceed 10% of your elected employee coverage.

Guaranteed Issue: Up to $10,000.

Eligible dependent children ages 14 days to 26 years (no age limit if full-time student).
